All registration items are due by 8/22/25 in order to begin school on 9/2/25.

How to Apply to MQP Preschool

• Registration Fee – it can be over the phone with card or in-person cash or check to secure your child’s spot.
• Complete Registration Packet (it is attached or can be submit a paper copy)
  • If all the paperwork is not completed by that date; the student will not be able to begin until the 2nd Quarter.
• Immunization Record
• Physical (ODJFS 01305 Child Medical Statement for Child Care Form)

How to Apply to MQP Kindergarten

• Fill out Inquiry Form
• IEP & ETR, if applicable – it can be over email or fax. This must reviewed by our Intervention Team prior to acceptance.
• Sign up for Kindergarten Screening – this is to split the students within their two homerooms
• Registration & Technology Fee – it can be over the phone with card or in-person cash or check to secure your child’s spot.
• Registration Form
• Scholarship Form
• Power of Attorney Form
• Submission of Documents: Proof of Address, Birth Certificate, Immunization Records, Baptismal Certificate (if applicable), and Custody Statement (if applicable)

How to Apply to MQP 1st through 8th Grade

First Step: Acceptance Process

• Fill out Inquiry Form
• IEP & ETR, if applicable – it can be over email or fax. This must reviewed by our Intervention Team prior to acceptance.
• Last Report Card – it can be sent over email, phone or fax.
• Recommendation Submission – Completed by Current School’s Teacher or Principal. The letter goes directly to our school for review (the link is below)

Second Step: Once Accepted by Principal

• Meet with our Principal
• Registration & Technology Fee – it can be over the phone with card or in-person cash or check to secure your child’s spot.
• Registration Form
• Scholarship Form – Only one scholarship form needs filled in
• Power of Attorney Form
• Request for Records Form
• Submission of Documents: Proof of Address, Birth Certificate, Immunization Records, Baptismal Certificate (if applicable), and Custody Statement (if applicable)

Registration & Technology Fees

Registration fees are due by 3/31 to receive $50 discount per child.

Each student pays a $25 technology fee.
